• First, complete the assessment on pages 139-141. This page does not have to be turned in to Canvas. 
• Second, once you get your score, refer to the score interpretation section in the readings that will help you answer the questions in the survey. 

Please refer to the rubric below for scoring information. Although this is automatically graded once you submit, we will review for substantive answers.

*Please note, if you do not answer the last two questions thoroughly, you will not receive full credit. There are no late points awarded for missed assessments.
